How to Add or Edit an Entity

You can add or edit entities for an un-enrolled provider, un-enrolled member, county, attorney, and others. You are also able to add an entity from a case or correspondence or other functions in the system. If the Add Entity process was requested from a case or correspondence record, then it is associated with the case record in the Case Regarding panel or the correspondence record in the Correspondence For or Inquiring About Entity panel.

When you are editing an enrolled provider or member, you are directed to the Provider Inquiry/Maintenance - Name/Address page or the Member Summary page, respectively, to perform your updates.

Optionally, you can:

To add or edit an entity

  1. If the Entity Search page is displayed because you have chosen to search for an entity as part of another task, continue to the next step.

    If the Maintain Entity page is displayed because you have chosen to add, edit, or view an entity from another task, continue to Step 3.

    Otherwise, on the Ct Mgmt menu, click Entity.

  2. On the Entity Search page, do one of the following:

    The appropriate fields are displayed on the Maintain Entity page based on the entity type. However, when you are editing an enrolled provider or member, you are directed to the Provider Inquiry/Maintenance - Name/Address page or the Member Summary page, respectively, to perform your updates. See How to View or Edit a Provider or How to Add or Edit Member Details.

  3. On the Maintain Entity page, to add or edit an entity's identifying information, in the Maintain Entity panel, complete appropriate the fields for that entity type:
  4. ClosedTo add or edit contact information, in the Contacts panel, complete the fields.
    1. Do one of the following:
      • To add, click Add Contacts.
      • To edit, in the Contact table, click the appropriate row.
    2. In the Contact panel, complete the fields.
    3. To store the information in the table, on the Contact action bar, click Save.
  5. ClosedTo add or edit address information, in the Address panel, complete the fields.
    1. Do one of the following:
      • To add, click Add Address.
      • To edit, in the Address table, click the appropriate row.
    2. In the Address panel, complete the fields.
    3. To store the information in the table, on the Address action bar, click Save.

      If the address cannot be verified, you have the option of saving the original address, choosing one of the various versions of the corrected address, or canceling the operation.

  6. ClosedTo add or edit phone information, in the Phone panel, complete the fields.
    1. Do one of the following:
      • To add, click Add Phone Record.
      • To edit, in the Phone table, click the appropriate row.
    2. Complete the fields.
    3. On the Phone Record action bar, click Save.
  7. ClosedTo add or edit e-mail information, in the E-Address panel, complete the fields.
    1. Do one of the following:
      • To add, click Add E-Address Record.
      • To edit, in the E-Address table, click the appropriate row.
    2. Complete the fields.
    3. On the E-Address Record action bar, click Save.
  8. On the Maintain Entity action bar of the page, click Save.

    When adding an entity, ND MMIS checks for duplicates. If there are potential duplicate entities already in the database, a warning message and a list of similar entities is displayed on the Duplicate Entry page. Continue to the next step.

    If no duplicates are found when adding an un-enrolled client, a CM Entity ID is created.

    When editing an entity, the information is saved.

  9. If there are potential duplicate entities, on the Duplicate Entity page, do one of the following
