The Entity Search page allows you to search for an entity. When the entity type is for a provider, the Entity Search page is displayed containing provider-related fields.
The Provider view of the Entity Search (Provider) page contains the following panels:

When the Entity Type is Provider, these fields are displayed. To add an entity, click Add Entity. |

Entity Type |
The type of organization, institution, or individual. Example: Provider |
Status |
Indicates whether the provider is enrolled or not. Active indicates that the provider is enrolled. Inactive indicates that the provider is not enrolled. Default: Active |
Entity ID Type |
The kind of ID entered in the Entity ID field. Example: In the case where Entity Type is Provider, then the Entity ID Type is Provider ID, NPI, SSN, etc. |
Entity ID |
The unique identifier used for an entity in ND MMIS about which healthcare information needs to be maintained. Note: Entity ID depends on Entity Type. For example, a Provider Entity Type requires a Provider ID. |
Provider Type |
The provider classification. Based on the Level I Provider Type of the Taxonomy Code Set. Examples: Dental, Pharmacy, Hospital, Vision, Physician |
Provider Sort Name |
Indicates how the providers are to be sorted. Note: Either Provider Sort Name or Last Name is required when Entity ID Type is not selected. |
Provider DBA | The name that the provider is doing business as. |
First Name | The first name of the provider. |
MI | The middle initial of the provider. |
Last Name |
The last name of the provider. Note: Either Provider Sort Name or Last Name is required when Entity ID Type is not selected. |
If more than one record is found, a table with matching records is displayed.
