Search Notes (Un-enrolled Provider) Page

You use the Search Notes page to search for notes for an entity. When the entity type is for an un-enrolled provider, the Search Notes page is displayed containing un-enrolled provider-related fields. Multiple notes may exist for a single un-enrolled provider. After the note details are displayed, you can filter, view, and print notes associated with the un-enrolled provider.

The Un-enrolled Provider view of the Search Notes page contains the following panels:

 

Search Filters Fields

Field Description
When the Entity Type is Un-enrolled Provider, these fields are displayed.
Entity Type

The type of organization, institution, or individual.

Example: Un-enrolled Provider

Entity ID Type

The kind of ID entered in the Entity ID field.

Example: In the case where Entity Type is Provider, then the Entity ID Type is Tax ID, NPI, SSN, etc.

Entity ID

The unique identifier used for an entity in ND MMIS about which healthcare information needs to be maintained.

Note: Entity ID depends on Entity Type. For example, a Provider Entity Type requires a Provider ID.

First Name The first name of the provider.
MI The middle initial of the provider.
Last Name The last name of the provider.
Organization Name

The name of the organization that the entity is associated with.

Note: Required if Entity ID Type is not selected.

 

Search Results Fields

If more than one record is found, a table with matching records is displayed.
