What Is Diagnosis?

A diagnosis code is a standard classification of diseases that is used to:

• Code and classify mortality (death) data from death certificates.
• Code and classify morbidity (disease) data from inpatient and outpatient records, physician offices, and most statistical surveys.

ND MMIS has the capability to handle both the International Classification of Diseases ICD-9-CM and ICD-10-CM diagnosis codes.

The International Statistical Classification of Diseases (ICD) is the international standard diagnostic classification for all general epidemiological, many health management purposes and clinical use, as classified by the World Health Organization (WHO). It is used to classify diseases and other health problems recorded on many types of health and vital records including death certificates and health records. The National Center for Health Statistics (NCHS), the Federal agency responsible for use of the ICD in the United States, has developed a clinical modification of the classification for indexing of medical records, medical care review, and ambulatory and other medical care programs, as well as for basic health statistics. 

ICD-9-CM, which stands for International Classification of Diseases, Ninth Edition, Clinical Modification, is organized into three volumes:

• Volume 1 (Tabular List)
• Volume 2 (Index to Diseases)
• Volume 3 (Index to Procedures and Tabular Lists)

In 1992, a tenth revision was completed titled: The International Statistical Classification of Diseases and Related Health Problems, 10th Edition, Clinical Modification (ICD-10-CM). It consists of a new alphanumeric coding that contains up to 7 alphanumeric characters of diagnosis classification information to report inpatient and outpatient conditions. It has not been officially released, yet. When released in October of 2013, ICD-10-CM will allow for more codes and greater specificity and thus better epidemiological tracking. This enhancement will allow providers to better identify certain patients with specific conditions that will benefit from tailored disease management programs, e.g. diabetes, hypertension, asthma.

Diagnosis Code in the Code Maintenance function of Rules Management North Dakota MMIS Web Portal provides the capability to define specific and non-specific diagnosis codes.

A series of indicators that are used to avoid costs when a third party may be responsible for paying for the services, limit services by diagnosis code and to facilitate detailed reporting can also be viewed, added, and edited. Whenever applicable, former related codes are displayed to support complex and historically dependent research needs.
