You can use the Member tab to search for referral requests using member information.
The Member tab of the Referral search page contains the following panels:

| Member ID |
Unique identifier used for a member and is assigned when the member becomes eligible to receive Medicaid benefits. |
| Begin Date |
The earliest date in the date range for which you are searching. Format: MM/DD/YYYY |
| End Date |
The latest date in the date range for which you are searching. Format: MM/DD/YYYY |
| Service Code Type |
Indicates whether the service code is a revenue code, procedure code or another type of code. |
| Service Code |
The revenue or procedure code for which the referral is submitted. |
| Mod1 - Mod4 |
Modifiers 1 - 4. A two-digit code attached to the procedure code to modify or clarify the description of the procedure. Up to four modifiers may be submitted for each procedure code, if applicable. |
| Provider Type |
Indicates whether the search is for the referred to or referring provider information. |
| Provider ID Type Code |
The type of provider ID. Examples: Medicaid, National Provider ID (NPI) |
| Provider ID |
Unique identifier used for a provider of healthcare services. Format: Up to 15 alphanumeric characters |
| Referral Type |
The type of referral. Examples: CSP, PCCM, Other Referral - Dental |
| Service Requested |
The specific service requested based on the type of referral. Examples: Hearing Aids, Outpatient Hospital Services |
| Location |
The work location assigned to the referral request. |
| Only Show My Referrals |
Limits the search results to only referral requests that have your user ID stored in any field on the referral record. |
If more than one record is found, a table with matching records is displayed.
