You use the Claims Inquiry Search Provider tab to search for claims using provider information.
The Provider tab of the Claims Inquiry Search page contains the following panels:

Provider Role |
Indicates whether the search criteria are for the billing or rendering provider information. |

Provider ID Type Code |
The type of provider ID. Examples: Medicaid, National Provider ID (NPI) |

Provider ID |
Unique identifier used for a provider of healthcare services. Format: Up to 15 alphanumeric characters |
|
DOS Begin |
Date of service begin. First date of service Format: MM/DD/YYYY |
|
DOS End |
Date of service end. Last date of service. Format: MM/DD/YYYY |
|
Media Source |
Indicates how the claim was received. Examples: Paper, Web |
|
Billed Amount |
The amount billed. If the amount includes cents, you must enter the decimal point. |
If more than one record is found, a table with matching records is displayed.
