Use the 1099 Inquiry page to search for and view 1099s associated with a particular provider. You must have Adobe Acrobat Reader installed to view the 1099 reports.

1099 Information Fields

Field	Description
Provider ID	Unique identifier used for a provider of healthcare services. Format: Up to 15 alphanumeric characters
Provider Tax ID	The provider's employer identification number.
Provider Name	The provider's name.
SSN	A nine-digit number assigned by the Federal government, usually assigned at the time of birth and used for tax purposes. Format: NNN-NN-NNNN or NNNNNNNNN Example: 123-45-6789

Provider 1099 Inquiry Fields

Field	Description
1099 Results Specifications Fields These fields display all the providers associated with your search criteria. Click a row to display a list of 1099 reports associated with that provider ID.
Provider ID	Unique identifier used for a provider of healthcare services. Format: Up to 15 alphanumeric characters
Provider Tax ID	The provider's employer identification number.
Provider Name	The provider's name.

Search Results Fields These fields display the 1099s associated with the selected provider ID. To view a PDF of the 1099 report, click the appropriate row.
1099 Year	The year covered by the 1099 report.
1099 Amount	The amount from Box 6 of the 1099.
View 1099 Image	The ID of the report's image.
