How to Apply For Medicaid

You must fill out an Application for Assistance to apply for Medicaid. You can get the Application for Assistance from the Department of Human Services (DHS) Web site. To access the Web site, do one of the following:

  1. On the Member menu, click How to Apply. The Medicaid page of the North Dakota Department of Human Services Web site is displayed.
  2. Select Apply for Medicaid and then click Application for Assistance.
  3. Print the application. The Application for Assistance Guidebook contains information that may help in completing your application

You can also pick up an application in person by going to a DHS office. You can find a State Office by doing one of the following:

Please read all the information on the Application for Assistance before filling out the application. Additional documentation may be required and all forms are available on the DHS Web site.

Your application and supporting documentation is reviewed by a DHS representative to ensure that you qualify for the programs for which you have applied. The DHS representative will contact you for an interview as part of the eligibility process.

Medicaid eligibility usually begins on the day you have met all the requirements for the program you applied for and if your income falls within the specified resource limits.
