How to Find a State Office

Use the Find a State Office directory to locate a state office where you can apply for Medicaid as well as other state programs.

To find a state office

Do one of the following:
- On the Directories menu, click Find a State Office.
- On the North Dakota MMIS Web Portal, click Contact Us.
The North Dakota Contact the Department of Human Services page is displayed in a separate browser page.
Do one of the following:
- To contact the North Dakota Department of Human Services (DHS) office, choose one of the telephone, FAX or email options.
- To display a list of programs and services by name, click Services & Help in the navigation panel located on the left side of the page and select the service for which you need information.
- To display the contact information, maps and directions for service locations, click Direct Service Locations in the navigation panel located on the left side of the page and select the appropriate heading.
To close the DHS Web site browser page, in the rightmost corner, click X.
