Search Procedure Code/Place of Service Page

You can use this page to search for procedure code/place of service rates.

The Search Procedure Code/Place of Service page contains the following panels:

 

Search Filter Fields

Field Description

To add a rate, click Add Procedure Code/Place of Service.

LOB

The unique code that identifies the line of business, a high-level payer, or funding source such as Medicaid.

Procedure Code

Indicates the procedure or service provided. Healthcare Common Procedure Coding System (HCPCS) or Current Dental Terminology (CDT) codes are typically used.

Can be used with the Starts With option.

Starts With

The Starts With option tells the system to retrieve all records that start with the search criteria entered. At least 2 characters must be entered when using the Starts With option.

Modifier 1 thru Modifier 4

A two-digit code attached to the procedure code to modify or clarify the description of the procedure. Up to four modifiers may be submitted on a claim for each procedure code, if applicable.

Note: If you select a modifier, you must complete the Procedure Code field.

Place of Service

Physical location where the health care service was provided.

Examples: Office, Urgent Care Facility, Inpatient Hospital, Independent Clinic, School, Ambulance - Land

If more than one match is found, this table is displayed. To view a match, click the appropriate row. None of these fields can be edited.

Procedure Code

Indicates the procedure or service provided. Healthcare Common Procedure Coding System (HCPCS) or Current Dental Terminology (CDT) codes are typically used.

Mod 1 thru Mod 4

Description

The short description for the procedure or procedure/modifier combination, if applicable.

Place of Service

Physical location where the health care service was provided.
