How to Recall a Provider Application

If you saved an incomplete application, you can recall it to complete it at a later time. You cannot recall an application that has been submitted. The North Dakota MMIS Web Portal will lock the application that you are recalling when you have entered its SSN/EIN incorrectly several times in a row. It will be locked for a specified amount of time for security purposes.

To recall a provider application

  1. Do one of the following:

    The Provider Enrollment page is displayed.

  2. In the Recall Provider Application panel, enter the Application Tracking # and SSN/EIN that was entered on your application.
  3. Click Submit.

    The Identifying Information page is displayed based upon the application type that you saved, either Individual Provider or Group Provider.

  4. To finish your enrollment process, complete any incomplete pages, depending on your application type:
